Changes to your Medi-Cal health plan

If you want Rancho to be your primary care medical home, you will need to join L.A. Care Health Plan and select a Rancho physician from the table below.

Starting in June 2011, most people with disabilities and seniors who have Medi-Cal only must enroll in a Medi-Cal Health Plan. This change is for anyone whose only insurance is Medi-Cal.

If you live in Los Angeles County, there are two healthcare plans you can choose from: Health Net and LA Care Health Plan. Both of these plans provide the same services as Medi-Cal, but with additional services at no or little extra cost.

If you are part of the Medi-Cal program, over the next year you will receive a form from the state of California to select the healthcare plan of your choice. The form will be sent before your birth month and must be sent back to the state by the end of your birth month.

Rancho Los Amigos National Rehabilitation Center is part of both healthcare plans and is considered a Primary Care Medical Home for members of the LA Care Health Plan. A Primary Care Medical Home at Rancho means improved communication and coordination of care with you and your physician, nurses, therapist and others in your healthcare team. If you want Rancho to be your primary care medical home, you will need to join L.A. Care Health Plan and select a Rancho physician from the table below.

I. Complete the Medi-Cal Choice Enrollment Form

You will find the Medi-Cal Choice Enrollment Form in your packet from the state. If you have lost it, you can go to the Health Care Options Web Site to download a new one. Use a blue or black ink pen to complete the form. Fill in as much information as you can. If you need help filling out the form, you can call Health Care Options, toll free at 1-800-430-4263 or use the linked sample with step by step instructions to complete your Medi-Cal Choice Enrollment Form.

II. Select a Rancho Doctor

Use the table below to select a doctor at Rancho. Enter the Doctor's 7-digit Clinic Code onto the Medi-Cal Choice Enrollment Form to indicate your choice.

Rancho's Primary Care Medical Homes

We have 4 primary care medical homes to serve you:

  1. Stroke Medical Home- provides primary care for people who have had a stroke.
  2. Spinal Injury & Spinal Disorders Medical Home - provides primary care for people who have had a spinal cord injury or have a spine disorder.
  3. Neurology Medical Home - provides primary care for people with developmental disabilities, seizure disorders, traumatic brain injury and other neurologic disorders.
  4. Diabetes Orthopedic Medical Home - provides primary care for people with diabetes related ulcerations, osteomyelitis, and/or amputations.

Member Services at Rancho Can Help

We're here to assist you. The Members Services Enrollment Office is located in Room 129-F in 100 Clinic Building on Rancho's campus. Stop by and visit us or call us at: (562) 401-7143. Office Hours are 8:00 am to 5:00 pm (Monday through Friday).

Member Services can assist you in the following ways:

  • Completing the enrollment form
  • Directing members to the correct primary care medical home at Rancho or within the Department of Health Services
  • Changing your Primary Care Medical Home
  • Providing resources such as access to our Wellness Program, Social Work, LA Care Resource Program, etc.
  • Providing authorization for inpatient or outpatient services
  • Providing authorizations for medical supplies and durable medical equipment
