Student Orientation Materials

Welcome Students! Please print and complete the following forms prior to your Live Scan appointment. Bring the completed forms and your driver's license, passport, or DMV Identification to your scheduled Live Scan appointment.

1. Employment Forms

Please review all policies and trainings, print and complete all forms. Do not sign or date. Forms must be signed at the time of your Live Scan.

  1. Nepotism
  2. Workforce Member Identification Badge
  3. Parking Permit Application
  4. Conviction Disclosure Instructions
  5. Background Investigation Policy
  6. Background Investigation and Medical Examination Acknowledgment
  7. Personnel Information Sheet
  8. Non-County Workforce Comprehensive Policy Statement
  9. Compliance Awareness Training - log-in with your name to complete the online training. This training will take between 60-90 minutes to complete. Print and submit the completed Code of Conduct acknowledgment form at the time of your live scan.
  10. Code of Conduct Training - review and submit the Code of Conduct acknowledgment form.

2. Health Clearance Forms

  1. Non-DHS/Non-County Workforce Member Health Clearance Certification - complete and submit all forms in the packet. The first page needs to be completed by your doctor or a licensed health care professional. Forms E2 and B-NC are required. Submit K-NC Declination form if applicable.

3. Trainings and Orientation

Complete all trainings and orientations prior to your first day. Bring completed quizzes/acknowledgments on your first day.

  1. HIPAA Self-Study - review and submit a completed quiz found at the end of the guide.
  2. Orientation Review Handbook 2011 - review and submit a signed attestation form found at the end of the document.
  3. Infection Control Orientation Self-Study - review and submit a completed post test found at the end of the guide.
  4. Risk Management Employee Handbook - review and submit a signed acknowledgment form found at the end of the document.
