Appointments and Referrals
In order to make an appointment or initiate a referral, please call the office manager at
(562)401-6800 during normal business hours, 8:30 am to 4:30 pm, Monday through Friday. A cover letter
and questionnaire will then be mailed to you. The cover letter lists information we need prior to
scheduling an appointment and the questionnaire helps assess your needs and expectations prior to your
appointment. Anyone can initiate a referral. We do, however, need a prescription from the client's
primary physician in order to provide services.
In order to provide the best services to our clients, it is important for us to have as much
background information on the client as possible prior to the appointment. The following information is
helpful and may be requested prior to the appointment: current medical summary written reports from
speech, occupational, and physical therapy, and Individualized Education Plan for school-aged clients.
Please be sure to download an intake form, fill out the
pertinent information, and follow the instructions as detailed on the right hand panel of this page for
submitting it.
